Dr Paw’s Pet Therapy Testing With Terry At Botsford Hospital 28050 Grand River Ave. Farmington Hills MI If you are interested in getting your dog tested I will be doing dates in December. You will need to FAX your completed forms to me and request for your FIRST date to test on the cover sheet. (All forms are on the first page of Dr. Paws - Michigan This will be a first come first served basis. This will be your first test and then we will set up future testing at a later date.You must read through all the information on the web site, watch the video and then go to Therapy Dogs Inc. and familiarize yourself with everything you are required to know.This does take some time so do a good job on your homework. We as testers, through Therapy Dogs Inc can only do 3 dogs at a time. So please, if you are unsure of attending please do not sign up. Please fax your forms below to 248-888-7488
Testing Dates Available Are
Please call hotline at 248.888.7488 before you come to make sure there are no changes to the schedule. We will meet outside the ER entrance. Please be sure to bring all forms and your 3 page test. Be early to settle your dog down before coming inside. I will confirm your appointment with an email when I receive your paperwork via fax.
